We look forward to working with the National Guard on Monday, May 24, to support vaccination of students and the community at Crossett Brook Middle School.
We’d like to provide a few reminders.

*Please note that this is the first of two Pfizer vaccine doses required to complete the vaccination process. A second dose of the Pfizer vaccine will be scheduled approximately 21 days after the first dose. It is anticipated, but not yet confirmed, that the second dose clinic will be scheduled on June 14 at CBMS.
The best way to reserve a dose of vaccine for your child is to register online. As part of registering online, you will be asked to provide your consent as their parent/guardian for your child to receive the vaccine. Children can bring one adult with them to the vaccination clinic. In special circumstances, additional support persons are allowed.
If your child attends Crossett Brook Middle School and you register for an appointment during the school day, your child will be able to attend their vaccine appointment while at school. Please note that Kathryn Saunders, the CBMS nurse, will be on site during school hours. Following 3:15, the school nurse will not be able to support getting your child to the clinic.
If your child attends the clinic without you, they will be asked to confirm the answers to health questions that were part of the registration process, such as whether or not they:
have any history of allergic reaction to vaccines or injectable therapies
are currently sick with COVID-19
have recently had any other vaccines
have a bleeding disorder or are taking blood thinners
are breastfeeding or lactating
There will be a limited number of walk-in appointments. We ask that you register ahead of time for an appointment. If your child takes advantage of a walk-in appointment, you will need to give your consent for them to be vaccinated in one of three ways:
You can complete and sign the Vaccine Consent Form AND Prevaccination Checklist.
You can come to the clinic and give your consent in person.
A member of the vaccine clinic staff can call you on the phone to talk to you and document your consent. (The HUUSD Nursing Staff does not recommend this method and requests that you submit written consent using the Vaccine Consent Form)
If our school clinic is not a convenient time for your family, there are many options statewide to get vaccinated. Your 12-15 year-old can register for a vaccine at any community vaccine clinic providing the Pfizer vaccine that is convenient to you. In addition, pharmacies at Kinney Drugs, CVS, Costco and Hannaford’s are now offering appointments for Pfizer vaccine to Vermonters aged 12 and up. Please note that each pharmacy has its own process and requirements for parental consent.
Below is a list of other confirmed Pfizer vaccine clinics in Washington County in the next few weeks. You may sign your child up using the online registration tool for any of these community vaccine clinics if the Crossett Brook Middle School date and time does not work for your family.
